Electronic Medical Record Submission


Please enter your QIO ID, Electronic Medical Record Key, and select your Electronic Medical Record file to submit.

  1. The QIO ID is 13 characters including dashes. Letters are UPPERCASE.
  2. The EMR Key is six UPPERCASE characters.
  3. The file must end in .pdf with a lower-case file name.
  4. Verify all uploaded files are searchable.
  5. DO NOT upload password protected files. Doing so will delay case processing.
  6. DO NOT upload encrypted files. Doing so will delay case processing.
  7. The maximum file size is 1 GB. If your file is larger than 1 GB, we recommend splitting it into 10,000 page increments.
  8. Please submit a single complete medical record with the medical record request attached as the first page.

This page is for Higher-Weighted Diagnosis Related Group (HWDRG) medical records.



Ensure that the file is a complete medical record for a single patient/beneficiary.
